On or around May 2, 2023, Perry Johnson & Associates (“PJ&A”) detected unusual activity on its computer network, indicating a data breach. Based on a forensic investigation, PJ&A confirmed that cybercriminals infiltrated its inadequately secured computer systems between March 27, 2023 and May 2, 2023.

PJ&A further determined that through this infiltration, cybercriminals accessed the sensitive personal information of tens of thousands of patients, including:
- Names
- Social Security numbers
- Dates of birth
- Addresses
- Medical information
- Insurance information
As a result of the data breach, these individuals’ personal and highly confidential information may be in the hands of cybercriminals who can place the information for sale on the dark web or use the information to perpetrate identity fraud.
