On August 7, 2024, Omni Family Health became aware of suspicious activity on its computer network, indicating a data breach. Based on a subsequent forensic investigation, Omni Family Health determined that cybercriminals infiltrated its inadequately secured computer systems and accessed its confidential files. The investigation further determined that, through this infiltration, cybercriminals acquired files containing the sensitive personal information of thousands of individuals and posted that information on the dark web.
The personal information exposed in the data breach includes:
- Names
- Addresses
- Social Security numbers
- Dates of birth
- Medical information
- Health insurance information
As a result of the data breach, these individuals’ personal and highly confidential information may be in the hands of cybercriminals who can use the information to perpetrate identity fraud.
