On March 27, 2022, First Choice Community Healthcare, Inc. (“First Choice”) became aware of a likely data breach involving its network. Based on a forensic investigation, First Choice confirmed that cybercriminals had infiltrated its systems and gained unauthorized access to certain files, including files containing patients’ personal and protected health information. According to First Choice, cybercriminals accessed or acquired the personal and protected health information of 101,541 individuals.

The personal and protected health information compromised in the data breach includes:
- Names
- Social Security numbers
- Diagnosis and treatment information
- Health insurance information
- Provider information
- Dates of birth
As a result of the First Choice data breach, these individuals’ personal and highly sensitive information may be in the hands of cybercriminals who can place the information for sale on the dark web or use the information to perpetrate identity fraud.
