On or around September 11, 2024, CDHA Management, LLC and Spark DSO, LLC dba Chord Specialty Dental Partners (“Chord”) identified suspicious activity on certain email accounts, indicating a data breach. Based on a subsequent forensic investigation, Chord determined that cybercriminals infiltrated its inadequately secured computer environment and accessed Chord’s data files between August 19, 2024, to September 25, 2024. The investigation further determined that, through this infiltration, cybercriminals accessed and stole files containing the sensitive personal information of thousands of individuals.
The information exposed in the data breach includes, but is not limited to:
- Names
- Dates of birth
- Social Security numbers
- Bank account Information
- Payment card information
- Driver’s license numbers
- Medical information
- Health insurance information
- Addresses
As a result of the Chord data breach, these individuals’ personal and highly confidential information may be in the hands of cybercriminals who can place the information for sale on the dark web or use the information to perpetrate identity fraud.
